    Since the beginning, Pathways has matured and grown significantly in the depth of its services and the size of its service area. As opportunities and needs present themselves, Pathways will always dedicate its resources to programs and services that provide the best outcomes for children & families. Care coordination services are an important aspect of care delivery because they connect people to resources that support the broader social determinants of health. Community CarePort specifically assesses all aspects that contribute to wellness and provides standardized Pathways that help people access and obtain the help they need.

    The site coordinator fills a pivotal role as the single point of contact working inside the school coordinating and providing integrated student supports. They work with school leadership and staff to connect students and families with community resources that help to address both academic and nonacademic needs, allowing students to show up. As Caltrans managers charged with annual pavement condition data collection for a 50, lane miles network, we are faced with a variety of challenges, including tight timelines, data quality audits, and constant coordination.     Pathways Center offers Outpatient Treatment programs in six of the counties that it serves.
